By submitting this application, I acknowledge that I am bound by the terms and conditions outlined below and that all information provided in this application is true and accurate to the best of my knowledge. I understand that if selected to participate in the photoshoot as a model, I am required to pay a $200 date holding fee, which will be applied as a credit towards the purchase of a collection. The reveal and ordering session will take place following the photoshoot. I further acknowledge that the $200 date holding fee is non-refundable in the event that I cancel or fail to attend the photoshoot. I understand that selection is subject to an application process and availability of dates. I acknowledge that there is a possibility that I may not be selected to participate in the model call. However, if selected, I agree to arrive on time for the photoshoot and the reveal and ordering session. I also agree to sign a model release allowing ShaBo STUDIO and it's immediate contractors (hair and make up artist) to share the images for advertising purposes.
